ワルファリン内服中の頭蓋内出血症例に対する第Ⅸ因子複合体投与の経験 Use of prothrombin complex concentrate for warfarin-associated intracranial hemorrhage 田村, 洋行 岡島, 真里 山岸, 利暢 八坂, 剛一 坪井, 謙 藤原, 俊文 ワルファリン 頭蓋内出血 第Ⅸ因子複合体 PT-INR warfarin intracranial hemorrhage prothrombin complex concentrates INR ワルファリン内服中に頭蓋内出血で救急受診した17人において，乾燥人血液凝固第Ⅸ因子複合体（PCC）とビタミンＫ投与の効果を検討した。外傷性５例，非外傷性12例，来院時プロトロンビン時間国際標準比（PT-INR）は，中央値 2.44（ 1.81- ９以上）であった。PCC を500IU または1000IU と，ビタミンＫを10mg または20㎎で投与した。PT-INR は10分後に中央値 1.47（ 1.05-1.9）に低下し，12-24時間後では，中央値 1.2（ 1.06-1.67）であった。PT-INR はPCC 投与により，急速に低下し頭蓋内血腫の増大は１例に認めたのみであった。退院時死亡例は３例（ 17.6％）であった。退院時の日本版modified Rankin Scale（ mRS） は，mRS ０ １例，mRS Ⅰ３例，mRS Ⅱ １例，mRS Ⅳ ６例，mRS Ⅴ ３例，mRS Ⅵ（死亡）３例であった。来院時modified NIH Stroke Scale（NIHSS）は中央値12（0-40）であった。mRS は外傷性出血かつ来院時NIHSS が低い症例で良好であった。PCC は頭蓋内出血におけるワルファリン効果の急速な是正に有効と考えられた。 Clinical findings and effects on blood coagulation of prothrombin complex concentrate（ PCC） and vitamin K administration during warfarin treatment of 17 patients（ 5 traumatic, 12 non-traumatic） with intracranial hemorrhagic complications were determined.The median（ range） international normalized ratio（ INR） at admission was 2.44（ 1.81 - above 9.00）； this decreased 10 minutes after PCC（ 500 or 1000IU） and vitamin K（ 10 or 20 mg） administration to 1.47（ 1.05 - 1.90）, persisting at 1.2（ 1.06 - 1.67） 12-24 hours later. INR decreased abruptly after PCC and vitamin K administration. In one patient with cerebral subcortical hemorrhage, a hematoma enlarged after administration, and 3 patients with non-traumatic hemorrhage died（ 17.6%）. Modified Rankin Scale values were as follows： 0 for 1 case, Ⅰ for 3 cases, Ⅱ for 1 case, Ⅳ for 6 cases, Ⅴ for 3 cases and Ⅵ for 3 cases. The median NIH Stroke Scale（ NIHSS） result was 12（ 0-40）. The Modified Rankin Scale at discharge was low for the traumatic patients and those having lower NIHSS results.
